Eight checks on unemployment-recognition Form 82

1. Recognition date and period: write Form 82 for every designated recognition date. Enter the attendance date and the period from the day after the previous recognition date through the current date exactly as the center specifies; do not reuse an old period.
2. Identity, account and notice: copy name, resident number, address and active phone. Enter the payment account at the first recognition or only when it changes; ask before using a seizure-protected account. Choose SMS or email for the payment decision and verify the destination.
3. All work and income: for the recognition period, mark yes for any public work, daily work, part-time work, artist activity, labor provision or other work, regardless of label. List what you did, every work date, income received, and promised income not yet paid. Mark no only after checking all records.
4. Business and other benefits: report business registration and any self-employment actually started even without registration, with start date and content. Report industrial-accident leave benefits received or expected and the benefit period; ask the center how overlapping benefits affect payment.
5. Job-search activity: for each activity, enter date, employer, phone, occupation, method and result. A pending, rejected or unsuccessful result is still recorded truthfully. Keep application receipts, emails, interview confirmations and screenshots showing the activity.
6. Other reemployment activity: complete self-employment preparation only when applicable; report confirmed employment or self-employment and its planned date. Mark qualifying center education, guidance, group counseling, training, volunteering, unfair-dismissal petition or another accepted activity and retain its evidence.
7. Attachments and optional sections: submit the eligibility certificate. When applicable, attach a diagnosis, employment certificate, contract, interview confirmation, activity statement or training certificate under Articles 90 and 91. Handle National Pension choices only if not already filed, and read the separate optional three-year personal-data consent before signing.
8. Declaration and result: review every page, date and sign the truth declaration. The form lists five-day processing and warns that false statements may cause suspension, repayment, additional collection and criminal penalties. Keep the filed copy, evidence, recognition days, payment calculation, amount and next date.

Example only — do not submit: Lan T.’s center designates 26 August 2026 and confirms the recognition period as 30 July–26 August. Lan reports a four-hour part-time shift on 12 August, lists the work and expected pay that has not yet arrived, and attaches the shift record. She enters two applications and one interview with truthful pending/rejected results and their emails, marks no business and no industrial-accident benefit. The example then leaves the unchanged bank account blank under the instructions, chooses SMS notice, signs, files, and keeps the decision and next date. Replace every fact, date, amount and activity with verified records.
