How to Claim Nursing Benefits After a Work Injury in Korea

Eight checks on KCOMWEL Forms 12-2 and 12-3

1. Confirm eligibility and timing first: nursing benefit is for actual care after medical recovery, not the care expense incurred while treatment is still ongoing. Check the recovery date, disability decision and whether renewed treatment affects the claimed days.
2. Claimant, account and period: enter the worker’s verified identity, accident date and receiving account. Mark whether the account changes and ordinary or benefit-only account. State exact start and end dates; do not include days without actual care.
3. Caregiver identity and relationship: enter name, birth date, address, mobile number, relationship to the worker and qualification. Choose spouse, child, parent, relative, other or the applicable professional qualification from real records.
4. Place and cost of care: select home, care hospital or another facility. For a facility, enter its name, location and paid cost. Free admission is not payable; paid-facility cost and records may be checked.
5. Other services and compensation: answer whether another law provides long-term-care, disability-assistance or another care service. Separately disclose damages or compensation for the same accident with date, amount, payer and evidence.
6. First claim versus later claim: submit Form 12-3 with the first Form 12-2 claim. The regulation allows the assessment opinion to be omitted from the second claim onward, but a changed condition or an office request can require updated medical evidence.
7. Doctor’s Form 12-3: the medical institution records accident and recovery data, disability grade, independence in eating, washing, bathing, toileting, dressing, walking and transfers, use of aids, paralysis, neurological or mental findings, and the overall opinion. The claimant must not complete or direct these medical findings.
8. Signature, change reporting and result: review optional automatic-payment wording separately, date and sign claimant or representative fields, and keep a copy and receipt. Report any caregiver or place change immediately; confirm whether automatic payment continues and preserve the written payment decision and any reassessment notice.

Example only — do not submit: Rafi K. completed treatment on 14 July and actually received home care from his spouse from 15 to 31 July. For his first claim he enters only those 17 days, identifies the spouse and home-care place, reports no paid facility or other-law care, and uses his verified benefit account. His hospital completes Form 12-3 from the medical record. He does not copy the example’s dates, grade or findings and does not ask the doctor to alter the assessment.
