How to read Form 34-4
- Full name: copy the passport spelling and keep spacing and order consistent with the residence card.
- Sex, nationality and phone: mark one sex box and use a current number that the office can reach.
- Foreign resident registration number: copy all digits from the card; do not publish or share the completed form unnecessarily.
- New address in Korea: use the road-name address and exact building/unit details shown on the residence evidence.
- Residence type: choose own home, jeonse/monthly rent or Others. An accompanying family member who is not the owner/tenant marks Others.
- Residence proof: match your case with a lease, purchase contract, dormitory confirmation, accommodation confirmation, residence confirmation or accepted receipt.
- Administrative-information consent: if you do not consent, provide the relevant records yourself; read every signatory line before signing.
- Reporter, date and agent: date and sign the report; if another person files, complete the power of attorney and verify the authorized person’s identity.
Example only — do not submit: Somchai P. moved on 20 August, writes his passport name, registration number and exact road-name/unit address, marks monthly rent. The example then attaches the matching lease, consents to administrative-data sharing, signs on 22 August and keeps proof of filing. Replace every detail with your verified move and document.
