Korean medicine side effects: Korean medicine side effects are easier to report when the exact product, dose, frequency, dates, other medicines and symptom timeline were recorded at the pharmacy and after use.
Korean medicine side effects: This episode starts with pharmacy instructions and continues only to recording and reporting a suspected unexpected symptom. It does not recommend a medicine, dose change or causal conclusion.
Korean medicine side effects: What you will finish
You will confirm practical dosing instructions, preserve product details, recognize urgent boundaries and understand the symptom, product, course, other-medicine and reporter fields in the official form.
Read the report fields, preview it, download the official PDF, then open the official online source
The official patient-or-consumer report records an unexpected symptom and medicine information. Reporting a suspected event does not prove that the medicine caused it, and the form is not a request for diagnosis, emergency treatment or compensation. The first page covers symptoms and product use; the second page adds the person who experienced it, the reporter and completion guidance. Unknown non-required details may be left unknown according to the form’s note.
How to complete the patient or consumer report
- Symptom information: describe what happened in plain words, when it started, the time from medicine use, recovery status and serious outcome if any.
- Product name: copy the exact name from the prescription, pharmacy bag or package; do not substitute a guessed ingredient or similar brand.
- Dose and use: record one-time dose, frequency, start/end dates and duration as actually used.
- Action and course: record whether use stopped and what happened after stopping or any later re-use; do not change treatment merely to complete the form.
- Other products and person information: list medicines or products used at the same time and complete only the patient details requested by the current form.
- Reporter and submission: identify the reporter, review mandatory marks, keep a copy and use the official KAERS channel; protect unnecessary personal data.
Worked example: copy a medicine name and dosing dates from the bag, describe a rash and its start time, list other medicines, and record what medical advice or action occurred. Seek urgent care first for severe symptoms. File the report as a suspected event, not as a claim that causality is proven.

Seek emergency help for trouble breathing, facial or throat swelling, collapse, severe widespread rash, blistering, or other rapidly worsening symptoms. For non-emergency suspected reactions, contact a qualified clinician or pharmacist for individual advice.

Five-minute practice
Copy the exact product name, dose, frequency and start date from the prescription or package. If an unexpected symptom occurs, write the timing and course, seek medical advice when needed, then use the official report only with facts you know.
