Read the reverse-side instructions, identify both parties, separate the result requested from the dated reasons, list evidence, review personal-data consent and confirm the competent committee before filing.
How to read and complete the official form
- Applicant: enter the tenant’s current contact and service address exactly; keep the public article copy redacted.
- Respondent: identify the landlord or other responding party sufficiently for official notice, using verified contract details.
- Purpose of application: state the concrete result requested, such as completing a specified repair, without mixing the whole chronology into this box.
- Reason and chronology: list discovery, each repair request, replies, visits and the present condition in date order.
- Evidence and attachments: number the lease copy, dated photos, messages, inspection or repair records and any other objective material.
- Consent and signature: read the collection and use notice, check the appropriate choice, date and sign only after confirming every page.
