The Ministry of Health and Welfare and KHIDI form book contains a general patient consent sample and many other foreign-patient forms. It is a reference sample for medical institutions, not one universal form used unchanged by every hospital. The sample says consent should follow an explanation of the purpose and process, expected positive and negative effects, possible adverse effects and complications, prognosis without treatment, understanding, signatures, date and time.

How to read the general patient consent sample

  1. Patient and procedure: confirm whose consent is recorded and the exact test, treatment or procedure being discussed.
  2. Purpose and process: ask what will be done, why, in what order and by whom; connect the explanation to your current symptoms.
  3. Expected effects: separate the intended benefit from what is only possible or uncertain.
  4. Adverse effects and complications: ask which risks are common, serious or urgent and what action to take if they occur.
  5. No-treatment prognosis and alternatives: ask what may happen without the proposed option and what reasonable alternatives exist.
  6. Understanding, interpreter and signatures: request interpretation or translation, ask questions, then record the correct patient or representative, clinician, date and time only after understanding.

Worked example: present a dated symptom and medicine list, hear the proposed test explanation, then restate its purpose, steps, likely benefit, material risks and alternatives. The actual hospital must use its own current form and explanation; this reference PDF is not a blank form to self-submit to KHIDI.

1. State onset and duration

Use a date and time when possible instead of only saying ‘recently’.

Point precisely and say whether pain is constant, intermittent or moving.

A number is subjective but helps communicate change when you use the same scale.
