The official KLAC file combines a civil legal-aid application and consent. It records the applicant, other party, dispute, requested assistance, factual basis and documents used to assess eligibility and the case. Consultation can begin before a representation decision, which requires separate review.
How to complete the KLAC legal-aid application
- Applicant and contact: copy current identity, address and reachable contact; provide occupation, income or household details only in the fields and documents requested for eligibility review.
- Other party and case: identify the opposing person or entity, address and known contact. If a court or agency case already exists, copy the exact case name, number, court and deadline.
- Request, facts and desired result: distinguish the legal help requested from the chronological facts and the practical outcome sought. Use dates, amounts, documents and prior notices rather than conclusions alone.
- Eligibility records, evidence and consent: list ID and eligibility material separately from contracts, payments, notices and other case evidence. Read the information-use consent, submit copies unless instructed otherwise, retain originals and save the consultation and filing record.
Worked example: a tenant identifies the landlord and lease dispute, tells the counselor the response deadline, asks for the specific help needed, writes the deposit payment and notices by date, attaches the lease and bank transfer, adds only requested eligibility proof, reads consent, signs and keeps the receipt.
